iPhone 5s models - which one do I get?

I'm about to upgrade my iPhone 4 to a 5s. I'm thinking of leaving Verizon (current provider) and going to TMobile. Which model iPhone 5s should I buy? If I buy the CDMA model for use on TMO, I can leave TMO and return to Verizon if there is a problem with the service. However, I'm concerned...
