Things and info to bring for iPhone upgrade program

I got my reservation for Friday to pick up the black 128gb iPhone plus. It says you need to bring proof that I am the account holder of my carrier plan. How do I show proof of that? It says bring your current iPhone, but do I need to bring the charger cable and headphones too? It doesn't say...
