Calgary man's struggles with mental health issues and addictions, prompts inquiry after suicide
 

A fatality inquiry is reviewing the circumstances surrounding the death of 26-year-old James Reader in the hopes it can prevent similar deaths. The psychiatric in-patient jumped from a Foothills hospital parkade in March of 2015.
