My XT860 reverts to factory default

I've owned my Motorola XT860 for 3 months twice now I've plugged it in to charge and then later returned to find my wallpaper and all my app shortcuts reverted back to factory default. Anyone know why this might be happening? It's annoying to have to re-add all my frequently used app shortcuts...
